Your clinic schedules the patients and provides a general exam room with an exam table. Dr. Croxford brings the EMG/NCS equipment and necessary testing supplies.
Clinic days may be arranged on a recurring or as-needed basis throughout Northern Illinois and Chicagoland.
Testing is performed by Jason Croxford, DC, R.NCS.T. Final reports are reviewed and interpreted by Alexandr Goldvekht, MD.

- 1Prepare the referral
Provide a physician order, applicable imaging, and any other relevant medical records.
- 2Confirm authorization
Your clinic obtains any required insurance authorization and confirms coverage before testing.
- 3Schedule the clinic day
A minimum of three patients is required. Appointments are scheduled every 30 minutes.
- 4Receive the report
Final reports reviewed and interpreted by Alexandr Goldvekht, MD are returned by secure email within two business days.
Billing and case types
Your clinic remains in control of the patient workflow.
Billing is performed by your clinic. Testing is available for personal-injury, workers’ compensation, and commercially insured patients.
Typical bilateral-extremity coding may include CPT 95912 and CPT 95886 billed with two units. Coding must reflect the services actually performed and applicable payer requirements.
