What does our clinic need to provide?
Dr. Croxford brings all EMG/NCS equipment and necessary testing supplies. Your clinic provides a general exam room with an exam table.
How many patients are required for a clinic day?
A minimum of three patients is required to schedule a clinic day.
How much time should we schedule for each patient?
Upper- or lower-extremity examinations are scheduled in 30-minute appointments.
Can we arrange recurring or occasional testing days?
Yes. Clinic days may be scheduled on a recurring or as-needed basis, depending on your location and patient volume.
Who schedules the patients?
Your clinic schedules its patients and coordinates appointment times for the clinic day.
Which services are performed on-site?
Dr. Croxford performs nerve conduction studies and needle EMG evaluations for upper- and lower-extremity clinical questions.
Who performs the testing and interprets the report?
Testing is performed by Jason Croxford, DC, R.NCS.T. Final reports are reviewed and interpreted by Alexandr Goldvekht, MD.
How quickly are reports completed?
Physician-interpreted reports are typically returned to your clinic by secure email within two business days.
Who handles billing for the studies?
Billing is performed by your clinic. Typical coding for bilateral-extremity testing may include CPT 95912 and CPT 95886 billed with two units. Coding must reflect the services actually performed and applicable payer requirements.
Which types of cases and insurance are accepted?
On-site testing is available for personal-injury, workers’ compensation, and commercially insured patients.
What information is needed before testing?
Please provide a physician order and any applicable imaging reports before the scheduled examination.
Where is clinic coverage available?
Clinic coverage is available throughout Northern Illinois and the greater Chicagoland area.
